This form is just to make it easier on our customers whether are individuals or companies who want to pay us for services used by their beloved one, Employees, friends, family members who are stranded out there or in need of taxi service, emergency lockouts or just flat tire…..etc
As everybody’s do know that companies cannot take a credit card numbers over the phone for security purposes, and because we are unable to check the ID’s to verify the person who is authorizing the charge for his/her credit card, therefore we came up with this idea to help us identify the person paying for the service, its just one single page needs to be filled out and sent along with Drivers License copy ( front & back ) & Credit Card copy ( front & back) to our office fax number 512-354-7473 or by email to manager@nataxi.com once the billing department have all the submitted docs approved & verified with the bank the office will be approving the service to be rendered.
Note:
The form has to be filled out clearly & signed by the person shown on the Drivers License.
The name on the credit Card submitted has to match the name on the drivers License.

2- Scan your credit card front and back along with your ID
3- take both pages and fax them to : (512) 354-7473
